Measuring Up - Performance Counts

Saint Francis HealthCare Partners (SFHCP) mission is to ensure the continual improvement of quality and patient outcomes, and the efficient management of clinical and financial resources. To aid in tracking performance in these areas, SFHCP develops and provides measurement tools and research studies.

Patient Satisfaction Survey

It is critical for physicians to gather data on patient satisfaction to help measure quality improvement and patient outcomes.  This data is used to support NCQA recognition programs and PQRI and, of course, to improve the quality of patient care.

In the spring of 2010, SFHCP will assist physicians in gathering patient feedback.  Practice offices will receive patient survey forms and a receptacle for collection of completed surveys.  SFHCP will pick up the receptacles, compile and analyze the survey results and provide feedback to the physicians.

Study to Measure Diabetes Management

SFHCP developed a research study to "Assess the Quality of Diabetes Management through the Use of a Personal Health Record."  With funding from the Connecticut Health Foundation, SFHCP has enrolled 18 physicians in this study, and is seeking to enroll as many diabetic patients as possible. The study continues through January 2012.
