Saint Francis HealthCare Partners Foundation

In 2006, Saint Francis HealthCare Partners (SFHCP) developed a non-profit foundation (the "Foundation") for supporting advanced quality initiatives including education, research, and the demonstration of better care outcomes and patient centeredness.

The Foundation has received funds from several sources to support these quality initiatives:

  • Connecticut Health Foundation: $200,000 to conduct a feasibility study on using an on-line Personal Health Record to aid in the self-management of diabetes.
  • Aetna: $2,500 to support the process of achieving NCQA physician recognition
  • WellCare: $5,000 to support the process of achieving NCQA physician recognition

In addition, the Foundation receives funding from outside entities to support the Annual Medical Conference, an educational program that qualifies for Continuing Medical Education (CME) credits.
