Saint Francis HealthCare Partners Foundation
The Saint Francis HealthCare Partners (SFHCP) developed a non-profit foundation (the "Foundation") focused on improving the quality of care delivered to members' patients through research utilizing evidence-based practice guidelines and information technology tools. The Foundation's goal is to assist members in effectively meeting the quality indicators identified in the evidence-based practice guidelines and to further SFHCP commitment to continuous quality improvement.
The Foundation has received funds from several sources to support these quality initiatives:
- Connecticut Health Foundation: $200,000 to conduct a feasibility study on using an on-line Personal Health Record to aid in the self-management of diabetes.
- Aetna: $2500 to support the process of achieving NCQA physician recognition
- WellCare: $5000 to support the process of achieving NCQA physician recognition
In addition, the Foundation receives funding from outside entities to support the Annual Medical Conference.
