Clinical Integration
Saint Francis HealthCare Partners (SFHCP) has developed and implemented a Clinical Integration Plan that encompasses programs, systems (including information technology), and policies and procedures.
By coordinating the elements of its Clinical Integration Plan, SFHCP provides a system of coordinated care that extends across the continuum of healthcare services, from the physician's office to the Hospital and into the home, extended care facilities or outpatient settings.
Participation in the Clinical Integration Plan is required of all members of SFHCP for the overall improvement of the quality and efficiency of the delivery of healthcare services.
SFHCP helps its members improve the quality and efficiency of healthcare services through the use of evidence-based practice guidelines to measure and monitor the performance of both individual physician members and the aggregate SFHCP's network. SFHCP also uses mechanisms for modifying physician member behavior and assuring adherence to SFHCP network standards.
