SFHCP Care Coordinators assist patients by:
- Coordinating services and resources necessary to meet overall care requirements
- Providing tips for navigating the health care system
- Coordinating and communicating with the health care team, vendors and patients regarding care planning and the implementation of optimal recovery goals
- Assisting in short- and long-term care planning
- Identifying high quality and cost-effective care alternatives
- Providing emergency room visit follow up through telephone contact and referrals to physicians and services
